Stable housing is health care. Puget Sound Peer provides Foundational Community Supports (FCS) Supportive Housing services to help eligible Apple Health (Medicaid) participants find and keep community-based housing—with the skills, coordination, and advocacy needed to remain housed long-term.

What we do (FCS Supportive Housing)

Our Supportive Housing services focus on pre-tenancy supports and tenancy-sustaining supports, delivered through person-centered planning and practical, real-world problem-solving.

Pre-tenancy supports may include:

• Housing-focused functional needs assessment (preferences, location, accessibility, roommate needs, reasonable accommodations, and barriers).

• Housing search support and application navigation (including document readiness and benefit linkage that supports housing stability).

• Building a person-centered housing plan with measurable goals and steps.

Tenancy-sustaining supports may include:

• Ongoing care coordination and linkage to primary care, behavioral health, SUD treatment, crisis services, and natural supports—because housing stability requires whole-person support.

• Independent living skill-building (household management, budgeting, de-escalation/anger management coaching, routines, and tenancy responsibilities).

• Landlord/property communication support (as authorized/appropriate), conflict resolution, and crisis planning to prevent eviction.

• Regular review/update of a housing support & crisis plan based on current needs and barriers.

Important notes (plain-language transparency)

• FCS does not pay rent, deposits, room and board, utilities, or ongoing bills—it pays for the support services that help someone obtain and maintain housing.

• Services require FCS authorization and are built around tenant choice, rights of tenancy, and separation of housing and services (support is voluntary and not tied to keeping a unit).
